If you were Perm Banned, appeal by using the template below:
1) Your Minecraft username:
2) Your Twitter handle (If you have one):
3) Approximate date of your perm ban:
4) Reason why you were perm banned (Please do not lie):
5) Do you agree to all the conditions stated above? (Yes or No):
6) Please state one IP that you will be using to log into the server. If you do not know, go to www.whatsmyip.com. All the other IPs that you used to login to the server will stay permbanned. Do not provide a VPN IP.
If you were an admin and your perm ban appeal gets accepted, you will be released and will be able to apply after 30 days for reinstatement.
DO NOT POST YOUR APPEAL IN THE SHOUTBOX!
